Our camp forms are “fillable” PDF files and you can use your computer keyboard to enter information. Depending on your browser, you may need to download the forms before completing them.  Be aware that your mobile device may not support the filling of PDF file form fields. Find our forms by scrolling down and clicking on the various form links.

All camp forms must be printed so that any required signatures can be done the old fashioned way!  Once complete, please upload them to your online account at least two days before you check in at camp.  You will be prompted to do this by email.  If you miss the due date, make sure to bring them with you to camp!!


The following forms are required of all resident campers and adults to be presented at check-in on the first day of your session. There are two different medical form attachments.  Please make sure you submit the appropriate Medical Release Form for either Regular Summer Camp or Family Group Camp.

In regard to your camper’s health, each session is staffed by a licensed Registered Nurse.  Please be sure to note any special medical needs or conditions on the Health Form.

PARENT/GUARDIAN

CONSENT FORM

Consent must be given for any child who participates in Family Group Camp without their own parent/guardian present. In order for that child to join a family group, the Consent Form must be signed by both parent/guardian and adult chaperone.

Venture 2 campers (grades 9 and 10) will participate in a day river trip which will require vehicle transportation off site. Please complete the Permission Slip form so that your child can participate.

If your high school aged camper is registered for Adventure Trip Camp, we require these two forms in order for them to participate in off-site adventure activities.
