Our camp forms are “fillable” PDF files and you can use your computer keyboard to enter information and your digital signature. Depending on your browser, you may need to download the forms before completing them. Be aware that your mobile device may not support the filling of PDF file form fields.

Not every form is necessary! Please submit only those forms that relate to the camp(s) for which you have registered.
You have the option of uploading completed forms to your online account with Active Network which was created when you registered for camp, and you will be prompted to do this by automated emails. However, we would prefer that you print and bring them with you to camp.
The following forms are required of all resident campers and adults to be presented at check-in on the first day of your session. There are two different medical form attachments. Please make sure you submit the appropriate Medical Release Form for either Regular Summer Camp or Family Camp.
Consent must be given for any child who participates in Family Camp without their own parent/guardian present. In order for that child to join a family group, the Consent Form must be signed by both parent/guardian and adult chaperone.
The forms below are required only for select camper sessions:
Venture 2 campers (grades 9 and 10) will participate in a day river trip which will require vehicle transportation off site. Please complete the Permission Slip form so that your child can participate.
If your high school aged camper is registered for Adventure Trip Camp, we require this from in order for them to participate in off-site adventure activities.
ADV – RESPONSIBILITY STATEMENT
Additionally, ACE Adventure Resort requires that their waiver be completed digitally from their website. We will reach out the week before camp and provide you the particular information needed to complete their form.
